Nurse Practitioner





LINES OF AUTHORITY: This position is supervised by the Medical Director. 


SMI  ?Children/Adolescent  Substance Abuse  DD

POSITION SUMMARY: The Psychiatric Nurse Practitioner provides outpatient and inpatient psychiatric services in a managed care setting to customers of ChangePoint programs, as directed by the Medical Director.


1.  Adhere to personnel policy and comply with agency requirements for  ethical and professional behavior.

2.  Provide medical services in individual and group format.

3.  Provide psychiatric assessment and consultation services.

4.  Make diagnoses based on DSM-IV.

5.  Recommend treatment plans for psychiatric customers.

6.  Document charts and prepare reports as required.

7.  Provide documented case consultation to ChangePoint treatment teams/personnel.

8.  Review and approve treatment plans, SMI checklists and other required  forms on known customers.

9.  Advise Medical Director and/or CEO regarding development and review of  programs and policies that impact services.

10.  Provides direct supervision to the ChangePoint Staff Nurse and the Medical


11. This position requires positive and active support of the company, the  customer and the co-worker.

12. Must participate in quality management and quality improvement activities.

13. Promotes quality and safety to help maintain a safe and pleasant work

  environment, follows safety regulations, and actively contributes towards a

  safe workplace.

14. Perform other duties as may be required by the Medical Director and/or  CEO.


Frequent interaction with customers, personnel and peers. 

Interface regularly with medical personnel, program directors and clinical  team members.

Collaboration, as appropriate, with other community resources/referral   agencies, with the customer's consent.


 Qualifications And Experience


Posses current license from Arizona Board of Nursing and Certification as  a Arizona Psychiatric Nurse Practitioner or a Arizona Family Nurse  Practitioner with psychiatric experience.

Previous clinical experience in community mental health and managed  care.

Current Arizona Driver=s License. 

Fingerprint Clearance.

Comply with a Drug Free workplace.

For Those Working With SMI:

 1.  Knowledge of symptoms related to qualifying diagnosis.

2.  Ability to identify and communicate signs of regression and   deterioration.

3.  Knowledge of interventions to foster self care and ADL skill  development.

For Those Working With Children/Adolescent:

1.  Knowledge and training in development stages and interface of  development with childhood disorders.

2.  Knowledge of Behavior Management and ability to develop age-  appropriate interventions.

3.  Able to communicate at an age-appropriate level.

For Those Working With Substance Abuse:

  1.  Able to identify signs and symptoms of Substance Abuse and Chemical  Dependency.

  2.  Knowledge of range of treatment for dependence including relapse,    prevention, strategies and self help groups.

3.  Able to provide a diagnostic impression per DSM IV criteria for   Substance Abuse and Chemical Dependency.

4.  Knowledge of history of substance dependence and of the   biopsychosocial influences and effects of dependencies.

For Those Working With DD:

1.  Knowledge of symptoms related to qualifying diagnosis.

2.  Ability to identify and communicate signs of regression and   deterioration.

3.  Knowledge of Article 9 (customer rights) of State requirements.

4.  Knowledge of Customer Intervention Techniques relative to self care  and ADL skills development.




Able to work within a multi disciplinary structure.

Strong verbal and written communications skills.

Trained in and effectively manages crisis intervention/behavior  management.

Strong organizational and time management skills.

Fluent in English both verbally and in writing. 


Able to effectively employ analytical and problem-solving skills.

Ability to develop professional reports, treatment plans and other clinical  documents in a professional and timely manner.

Ability to interpret and comply with policies and regulations, and other  applicable state and federal requirements.

Ability to work cordially with personnel, customers, community agency  personnel, and the public.

Able to work with complex administrative systems in an environment of  limited funding and cost-containment practices.

Able to travel.


 Physical Requirements

Able to sit and stand for extended periods of time, less frequently,  walking, lifting and reaching.

Able to hear ordinary conversation and phone communications.

Visual capacity necessary to fulfill documentation, responsibilities of job.

Negative TB skin test. 

 Equipment Operation

  Able to operate general office equipment.

Able to operate Company vehicles.

 Environmental Conditions

  Air conditioned and/or heated office setting.
