Lines of Authority

This position is supervised by the DD Program Manager or designee.

Position Summary

The DD Case Manager is responsible for assisting and/or training customers with developmental disabilities at our day location or out in the community in areas stated in their ISPP.

Duties and Responsibilities

1.  Adheres to personnel policies and comply with agency requirements for ethical and professional behavior.

2.  Provide a caring and encouraging environment for learning.

3.  Provide for the personal needs of the customers, which may occur during treatment.

4.  Treat customers with the dignity and respect as stated in Article 9 and all other DDD required trainings.

5.  Individualize each customer's program to meet his or her needs.

6.  Provide customer transportation in company vehicles.

7.  Communicate effectively with customers.

8.  Administer customer objectives and strategies.

9.  Complete and submit all DDD required documents (i.e., monthly summaries, data sheets, service notes, etc.), in a timely manner.

10.  Attend all customer staffing, ISPP, and personnel meetings.

11.  This position requires positive and active support of the company, the customer and the co-worker.

12. Promotes quality and safety to help maintain a safe and pleasant work environment, follows safety regulations, and actively contributes towards a safe workplace.

13.  Must participate in quality management and quality improvement activities.

14.  Performs other duties as assigned by the Program Manager or designee.

 

Primary Contacts

  • Frequent interaction with customers and their support system, usually their family, and other personnel.

Position Requisites

Qualifications and Experience

Required:

  • High School Diploma; or General Education Equivalency Certificate.

  • Must be able to complete and receive certification in all required training within six months of accepting position.

  • Current Arizona Driver's License.

  • CPR and First Aid certification.

  • Fingerprint Clearance.

  • Knowledge and ability to define the needs of the customer served.

  • Comply with a Drug Free workplace.

  • Must be at least 21 years old.

For Those Working With DD:

1.  Knowledge of symptoms related to qualifying diagnosis.

  2.  Ability to identify and communicate signs of regression and deterioration.

  3.  Knowledge of Article 9 (customer rights) of State Requirements.

4.  Knowledge of Customer Intervention Techniques relative to self-care and

  ADL skill development.

Preferred:

  • Six months experience in working with Developmentally Disabled.

  • CPR and First Aid certification.

  • Knowledge of entitlement process and local resources.

Skills/Abilities

Required:

  • Fluent in English both verbally and in writing. 

  • Literate.

  • Able to effectively employ analytical and problem-solving skills.

  • Able to travel.

  • Ability to work assigned shifts.

      Preferred:

  • Ability to meet deadlines for project completion.

  • Ability to organize and schedule work to maintain efficient work flow.

    Physical Requirements

  • Able to perform physical functions of the job with or without reasonable accommodation as necessary.

  • Able to communicate using auditory, visual, written tools or use other service aides for the completion of the work.

  • Able to visually see a computer screen and various kinds of written documents or use other service aides for the completion of the work.

  • Negative TB Skin Test.

     

    Emotional Capabilities

  • Able to work with customers who are Developmentally Disabled and require patience.

    Equipment Operation

  • Able to operate general office equipment.

  • Abe to operate company vehicle.

  • Able to operate vehicle wheel chair lift.

    Environmental Conditions

  • Cooled and/or heated office setting.
